Document upload link from other entity lists

Sage CRM provides document upload functionality against almost all the entities in CRM. We can use the same to upload Draft and Finalized versions of relevant documents. Recently we came across the requirement where we were supposed to show the upload link as the separate hyperlink column in custom entity list screen.
The requirement was to load list of services provided to customer and a hyperlink to upload the Agreement document against that particular service. If document is already uploaded, this link will display the document name.

Clicking on the document hyperlink shows below screen to upload the document.

Uploaded document is visible under the list against the service as shown below.
